Management hyperfocused on short-term financial metrics often to the detriment of staff and even long-term company interests at times. Limited career growth opportunities. High performers are not rewarded. Seniority and years of service are valued above all else. Salaries are very low for junior and mid-level staff.

PM's don't divide work properly (ie, a few extremely overworked, most begging for work) which leads to high turnover. No incentive for quality work but fast, uniform work corrected later is promoted.
