Unreasonable expectations / hours (60-70 hours per week)
Promotion process entirely based on your manager's willingness to nominate you, has very little to do with your work, firmwide reputation or accomplishments.
You have to work as a the next title up for a year (at your current title/pay) to "prove" it, and then they will put you up the following year (nominations in spring) for a promotion to take place in Jan. So effectively you have to work a level up for almost 2 years without being paid or recognized for it...and if you don't get nominated during the first cycle, you end up having to keep doing so for 3+ years.
