Senior management appear to be hiding their inabilities to manage effectively. When there are bad decisions made, they appear to sweep it under the rug and move onto the next bad decision. There is no recourse for bad decisions and lessons are not learned so knowledge is not passed on. The board don't seem to have a clear view of what is happening almost as though the senior management are filtering the information.
